To establish a wholesale credit account, please fill out this Credit Application form and complete your application by printing, signing, and sending or faxing the form to us. We will contact you about the account status as soon as possible. Note: credit accounts are available to wholesale trade customers only.

All of the fields on the Credit Application form are required, as described below:

Submitted By section asks for basic contact information about your business.

Ownership section asks for information about the type of company and the ownership. Please list all applicable company owners. Room is provided for up to fourprincipal owners; at least one is required. If you are the sole owner, list that information under "Principal 1."

Financial Information asks for information about your primary bank account. If you don't know the name of your Bank Officer or Department, enter "unknown."

References. Please include at least one trade reference through which you have an established account or credit. Room is provided for up to four trade references; at least one is required.

Confirmation. Please attest that the information you are submitting is accurate by signing your name, title and date.

Your information will be used only by Green Value Nursery to establish a wholesale trade credit account. Refer to our Privacy Statement if you have questions.
